Overview
Position Objective:
The Autonomous APRN plays a vital role in supporting Better Health Group's (BHG) Management Services Organization (MSO) by assisting affiliate primary care physicians in delivering high-quality care to Medicare Advantage patients within a value-based care model. Assigned to a specific region and clinic network, the APRN will be based in designated clinics each week, where they will: conduct Annual Wellness Visits (AWVs); provide direct medical care to patients; order and interpret diagnostic tests; develop and implement patient treatment plans; and perform chart reviews to ensure accurate and comprehensive documentation. This is a safety-sensitive position, as it involves direct patient care and medication administration. The APRN will report to a Medical Director and collaborate with both internal BHG teams and external healthcare professionals to ensure seamless, high-quality patient care while supporting overall clinical team functions.

Responsibilities include and are not limited to:
- Practices medicine autonomously using sound medical judgment while adhering to ethical and professional standards; seeks consultations when necessary and stays current with industry advancements and best practices.
- Provides comprehensive care for older adult patients with chronic conditions, ensuring accountability for patient outcomes; diagnoses and manages chronic diseases, such as Diabetes, Atherosclerotic Disease, COPD, CHF, obesity, etc. in the senior population; seeks specialist consultations as needed while maintaining involvement in patient care.
- Ensures compliance with all clinical obligations, including Accrediting Agency standards, CLIA, HIPAA, state and federal regulations, prescription guidelines, and safety protocols.
- Assesses, diagnoses, treats, manages, educates, and coordinates care for patients with complex and chronic health conditions using evidence-based medicine and practice guidelines.
- Follows all internal policies and procedures, ensuring timely completion of all operational tasks according to BHG's guidelines for clinical processing and documentation.
- Maintains complete and accurate patient records within the electronic medical record (EMR), ensuring all documentation is completed within 48 hours of each visit.
- Educates patients on health promotion, risk reduction, disease prevention, and chronic condition management.
- Analyzes patient histories, symptoms, physical findings, and diagnostic information to develop appropriate diagnoses and treatment plans.
- Interprets diagnostic results, (e.g., blood gases, radiology, EKGs) in collaboration with the supervising physician to determine appropriate care plans.
- Demonstrates professional behavior that emphasizes compassion, quality, loyalty, and conscientiousness in patient care.
- Maintains licensure and certification requirements, including completion of all required continuing education.
- Ensures patient confidentiality by adhering to HIPAA policies and procedures.

- Licensure & Certification:
- Must hold Autonomous Advanced Practice Registered Nurse (APRN) licensure in the applicable state.
- ACLS/BLS certification as mandated by state regulations.
- Education & Experience:
- Graduate of an accredited school of nursing.
- 3+ years of professional nursing experience.
- 3+ years of primary care nurse practitioner experience.
- Experience working with electronic medical record (EMR) systems.
- Language & Communication Skills:
- Bilingual (Spanish) required; must be proficient in reading, writing, and speaking.
- Excellent written and verbal communication skills with the ability to engage effectively with senior leadership, providers, and health plans.
- Ability to convey complex information clearly.
- Ability to read, analyze, and interpret professional literature, technical documents, and governmental regulations.
- Capable of effectively presenting information and responding to questions from diverse patients/audiences.
- Technical & Analytical Skills:
- Proficiency in Google Suite (Drive, Docs, Sheets, Slides) and Microsoft Office (Word, Excel, PowerPoint) for real-time collaboration.
- Strong critical thinking, problem-solving, and data analysis skills with the ability to collect and interpret information to make informed decisions.

Physical Requirements:
- Full range of motion, including handling and transferring patients, manual and finger dexterity, and eye-hand coordination.
- Ability to stand, walk, push, bend, kneel, and reach (both at arm's length and overhead) for prolonged periods in a clinical setting.
- Must be able to lift and move patients as well as medical equipment as needed.
- Ability to lift and carry up to 15 lbs.
- Corrected vision and hearing within normal range required.
