Position Objective:
The Care Coordinator serves as a liaison between Better Health Group clinical teams, Primary Care Providers, and external hospitalist providers. The position focuses on building and maintaining relationships with Hospitalists, Hospitalist Coordinators, Medical Directors, PCPs, Better Health Group clinical teams, and patients, to ensure smooth patient transitions and efficient and effective care coordination for patients that are in the Emergency Room or inpatient hospital settings.

Responsibilities
Position Objective:
The Care Coordinator serves as a liaison between Better Health Group clinical teams, Primary Care Providers, and external hospitalist providers. The position focuses on building and maintaining relationships with Hospitalists, Hospitalist Coordinators, Medical Directors, PCPs, Better Health Group clinical teams, and patients, to ensure smooth patient transitions and efficient and effective care coordination for patients that are in the Emergency Room or inpatient hospital settings.
Responsibilities:
- Actively supports all aspects of care coordination for a panel of Medicare Advantage patients that are assigned to Hospitalists and that provide services to multiple hospital systems
- Performs patient triage
- Develops relationships with key stakeholders to impact outcomes
- Actively communicates and handles care coordination, care transition, and other outreach to and from Hospitalists, Hospitalist Coordinators, Medical Directors, PCPs, Better Health Group clinical teams for patients that are in the Emergency Room, Observation or Inpatient Hospital settings
- Provides follow-up on patient care, discusses patient care with Hospitalists, Hospitalist Coordinators, Medical Directors, PCPs, Better Health Group clinical teams, and works to resolve any problems that may arise
- Serves as a point of contact for Better Health Group and maintain communication with the patient's family
- Ensures timely and effective communication and coordination between Hospitalist partners and Better Health Group
- Reviews daily ER visits and inpatient admission notifications and provides the necessary patient follow-up
- Escalates patient cases to the assigned Medical Director when needed
- Schedules PCP follow-up and other appointments and services
- Maintains accurate and timely documentation of all calls in the patient's Electronic Medical Record (EMR)
- Meets daily performance metrics established for care coordination and outreach activities
- Provides education to patients with chronic conditions on the importance of self-management and ensures patients are following instructions for upcoming provider visits
- Assesses barriers to discharge and access to care and supports necessary to prevent a readmission
- Provides follow-up contact with patient as indicated to ensure compliance with provider recommendations
- Meets regularly with the Manager, Care Coordination to review patient panel, day-to-day questions, and to address any clinical concerns/reviews
- Demonstrates a general understanding of managed care principles, goals, and targets
- Ensures compliance with HIPAA and other Federal and State required data policies
- Additional duties as assigned
Position Requirements/ Skills:
- At least one year of care/service coordination or case management experience
- Experience working as a Medical Assistant, Licensed Practical Nurse, Community Health Worker, or Health Coach preferred
- Experience working with Electronic Medical Records (EMR) software, specifically eCW
- Proficient computer/typing skills
- Proficient with Google Suite (Drive, Docs, Sheets, Slides) for real-time collaboration
- Advanced knowledge of medical terminology
- Experience in Primary Care
- Excellent documentation skills
- Proven ability to build trust and foster strong relationships with patients, co-workers, and stakeholders
- Ability to work in a fast-paced environment, demonstrating motivation and results-oriented focus
- Exceptional skills in communication, organization, multi-tasking, and project management
- Strong problem-solving, consultative, and critical-thinking abilities
- Comfortable working cross-functionally with multiple teams and independently with minimal supervision

- Demonstrated ability to handle data with confidentiality
- Bilingual is preferred
Physical Requirements:
- Ability to work on a computer for extended periods
- Capability to maintain a stationary position, either sitting or standing, for prolonged durations
- Effective communication skills to exchange information with others
- Repetitive motions involving the use of wrists, hands, and fingers
