Advanced Practice Registered Nurse - Virtual Program

Overview

The Virtual APRN plays a critical role within Better Health Group's (BHG) Management Services Organization (MSO), supporting BHG's wholly owned primary care clinics (VIPcare) who care for Medicare Advantage patients in a value-based care model. This position is responsible for delivering high-quality, collaborative care through a hybrid model that includes virtual visits across multiple markets. Focused on managing patients with complex and chronic conditions, the Virtual APRN works closely with internal BHG teams and external partners to support improved clinical outcomes through evidence-based care. This role reports directly to the Chief Medical Officer (CMO) and, while primarily remote, requires occasional travel to VIPcare clinic locations as needed.

Location: Based in Florida

Responsibilities include and are not limited to:

  • Performs health assessments and preventative measures within established protocols and under Physician guidance.
  • Orders, interprets, and evaluates diagnostic tests to assess patient clinical conditions and inform care decisions.
  • Conducts clinical procedures including phlebotomy, EKGs, blood glucose monitoring, INR checks, spirometry, ankle-brachial index testing, and vaccine administration in VIPcare clinics.
  • Participates in complex care discussions and case reviews with physicians.
  • Develops and documents comprehensive care plans in the Electronic Medical Record (EMR).
  • Prescribes medications or therapeutic interventions in accordance with clinical guidelines and state regulations.
  • Coordinates care delivery across remote environments.
  • Keeps Primary Care Physicians informed of patient status, interventions, and treatment plans.
  • Collaborates with specialists and interdisciplinary teams as necessary for integrated care.
  • May participate in virtual on-call rotations, which may include evenings, weekends, and holidays.
  • Maintains detailed, timely documentation, with all notes closed within 48 hours of patient encounters.
  • Performs other duties as assigned.


Position Requirements/ Skills:

  • Licensure & Certification:
    • Active APRN license in good standing, with willingness and ability to obtain additional state licensures as needed.
    • ACLS/BLS certification as mandated by state regulations.
  • Education & Experience:
    • Graduate of an accredited school of nursing.
    • 3+ years of professional nursing experience.
    • 2+ years of primary care nurse practitioner experience.
    • Familiarity with value-based care metrics and care gap closures.
    • Experience working in Geriatrics highly preferred.
    • Experience working in virtual care or with telehealth platforms preferred.

    • Excellent written and verbal communication skills with the ability to engage and educate effectively with patients, caregivers, and providers.

  • Technical & Analytical Skills:
    • Competence in performing basic clinical procedures such as phlebotomy and EKGs.
    • Experience working with electronic medical record (EMR) systems.
    • Proficiency in Google Suite (Drive, Docs, Sheets, Slides) and for real-time collaboration.
    • Ability to handle confidential data with discretion.
  • Clinical & Patient Care Skills:
    • Skilled in traditional nursing disciplines such as patient assessment, diagnosis, treatment planning, and management.
    • Strong understanding of chronic disease management, care coordination, and quality improvement principles.
    • Skilled in treating chronic conditions.
    • Ability to remain calm and effective in emergency situations.
    • Strong understanding of appropriate patient care rationale and evidence-based practice guidelines.

Physical Requirements:

  • Full range of motion, including handling and transferring patients, manual and finger dexterity, and eye-hand coordination.
  • Ability to stand, walk, push, bend, kneel, and reach (both at arm's length and overhead) for prolonged periods in a clinical setting.
  • Must be able to lift and move patients as well as medical equipment as needed.
  • Ability to lift and carry up to 15 lbs.
  • Corrected vision and hearing within normal range required.
  • Ability to sit for extended periods when performing administrative tasks.
  • Must be able to travel within the assigned territory as needed up to 5-10% of the time.
